1. If you have a printer, fill out our form below and bring it to Oliver's Market.
   
2. Once at Oliver's Market, ask a cashier for a Community Card, and give them your pre-filled form or ask for a blank form.
   If you fill out the form in-store, please check mark 'Peace & Justice Center of Sonoma County'.
   
3. Show your card each time you check out.
